Telehealth Update

We are offering video calling appointments moving forward! Telehealth appointments are available for individuals who need to speak with a medical or behavioral health provider over video calling. Patients will download and use the Zoom Mobile App.

Services

Medical Referal

Medical referral covers basic office visit (primary care), it is valid for one year from date issued. Items NOT covered are specialist/urgent care, emergency room visit, surgery/hospital stays/medical bills and prescription.

Eyeglasses - Adult (SOV)

Please bring a picture ID with you at the time of your visit. A blood “sugar” and blood pressure screening will be performed before an eye glass voucher is issued to determine eligibility. An eye glass voucher is issued every two (2) years to the date they were recieved. If an eye glass voucher is needed before the two (2) year date, it will cost $30 after the screening. Please bring in your immunization record, as well.

Adult eye glass vouchers (SOV) are ONLY available on Wednesdays by walk-ins, or schedule an appointment.

Wednesday 8:30 am- 11:00 am and 1:00pm- 3:30 pm

Eyeglasses for Kids

Kids eye voucher is issued every two (2) years to the date. If a kid eye voucher is needed before the two (2) year date, it costs $35 after the screening. If you have medical insurance that covers glasses you are not eligible for a free kids eye voucher. Kids will get their blood “sugar” and blood pressure screened. You will need to bring the child's immunization record.

Mon. & Fri. 8:30 am-11:00 am and 1:00 pm- 4:00 pm.
Tues. & Thurs. 8:30am- 11:00 am and 1:00 pm- 3:00 pm         Wednesday 8:30 am- 11:00 am and 1:00 pm- 3:30 pm

Immunization

Immunizations are services offered by our Community Nursing Services (CNS) or Community Health Clinic Inc. (CHC). Onsite immunizations are available and only for limited time.

Dental Referal

Dental referral covers preventative care only, such as: x-rays, cleaning, filling, simple extraction & root canals. A referral is needed for each appointment. Items NOT covered but client must pay out of pocket for crown, flipper/partial, denture & bridge (priced on a sliding scale).

Screenings

Cedar Point offers blood pressure, blood “sugar” and cholesterol screenings by walk-in or by appointment. If you are a walk-in there may be a wait time. Call to schedule an appointment.

 

 

Physical Activity

Physical Activity/Exercise is an important part of a healthy life style, especially for preventing and controlling diabetes, high blood pressure and weight. We offer physical activity instruction classes. Individual appointments with a Personal Trainer, may include design and instruct an exercise plan. Referrals to Occupational Therapy Clinic and Senior Center gyms. Call to schedule an appointment.

Gym Open mon-fri 8:30 am- 4:00pm

Cedar Point Wellness Center believes that good health includes styaing physically active. We have a fitness center located in the building, and it is available to all clients over the age 18.

Gym Equipment includes:

– Treadmill
– Weight Bench – Free Weights
– Resistance Bands
– Recumbent Bike
– Elliptical Trainer

Good nutrition is an important part of a healthy life, especially for preventing and controlling diabetes, high blood pressure and weight. We offer nutrition education for all registered members of the community. You can schedule a one-on-one consultation for yourself and your family. Try new recipes and learn how to improve family favorites with our community cooking demos. Contact our Registered Dietitian at 801-214-7667 for an appointment, or for more information.
