Registration can be completed online or in person during regular business hours.

When registering online or in person; the following documents will be needed:

  • Certificate of Indian Blood (CIB) or Tribal Identification card
  • Social Security Card
  • Current Driver’s License or Photo ID
  • Proof of Residency: a bill, rental agreement, or official mail with address.
  • Proof of Medical Insurance card (if applicable)
  • Verification of Income (if applicable, please bring paystubs)
  • All documents need to be submitted and recorded prior to receiving services at UICSL.

After all paperwork has been submitted online or in person, please allow 24 to 48 hours for processing. 

A potential UICSL client (child or adult) may be eligible to register for UICSL services as a descendant through a tribally enrolled parent or grandparent. Registrations via first or second descendancy require birth certificates that directly connected the person registering to the enrolled parent or grandparent along with the enrolled family members’ verification of tribal enrollment (CIB, Tribal ID, etc).

Each situation can be unique. Please call us at (801) 486-4877 to speak to our Patient Registrar to determine what is needed for you or your child to register for services.

Certificates of Indian Blood (CIBs) and Tribal IDs are issued by tribal nations or your tribal enrollment office. Please contact your tribal enrollment or vital records office for information on how to obtain a copy.

In short, for those who have no information on which family member has/had tribal enrollment, they will need to do some extensive family history research in order to gather vital records information to submit to a tribal enrollment office that proves direct lineage.  Again, requirements and eligibility for tribal enrollment vary by tribal nation. Contact the tribal enrollment office to know what is officially required.

There are no staff members at UICSL that help with ancestral research. Refer to the online resources to start your research. If you would like to hire a genealogist, search via Google for a local genealogist or family history researcher.

A Certificate of Indian Blood (CIB) or Tribal ID is required to register as a client at UICSL. DNA tests results do not meet the Tribal ID card or CIB requirement.

UICSL does not release any third-party documentation (Driver’s License, CIB, Social Security card, etc.) presented to our office when you register.

If you are coming to the UICSL for the first time, please bring the 4-6 items requested for registration – even if the registration is completed online. Once it has been confirmed that we have all required information in your chart, you will be eligible to receive services.

Please bring your photo ID and insurance card to every visit.

Some UICSL services do not require health insurance. However, services that might be needed outside of our facility may require payment by the patient.

To avoid delay in health services please meet with our Benefits Coordinator to discuss health insurance options, such as applying for Utah Medicaid.

If you are insured with Medicaid, Medicare or private insurance, UICSL may bill your insurance provider so that we may be able to provide services to more people in our American Indian / Alaska Native community.

UICSL offers healthcare access and case management services, diabetes support services, behavioral health outpatient services, family programs, social services, referrals and other services.

All registered clients of UICSL and their household members qualify for COVID-19 vaccines. If you are not registered, please register online or come into the office with your registration documents.

If a member of your household is not a tribal member, they may still be vaccinated as a registered non-beneficiary. This household member will have to come to the office to fill out some paperwork before a vaccine appointment can be scheduled.

Yes, all registered clients of UICSL can be tested for COVID-19. Please call (801) 486-4877 to connect with our Registered Nurse who will determine which type of test is most appropriate and to schedule an appointment.

We do not offer legal assistance at UICSL. If you need legal services, please contact Utah 211 by dialing or going to their website for legal resources in Utah.

UICSL is a medical clinic. Face Masks are REQUIRED in our facility, regardless of state or local mandates or vaccination status. Everyone will be screened for COVID-19 symptoms as they enter the building.

UICSL partners with local Community Health Centers to provide preventative medical and dental care services to established UICSL clients. Anything out of this scope could require the cost of services to be paid by the patient.

These services are available by referral from our Referrals office during office hours on a walk-in basis.

Limited medical services can also be obtained by making an onsite appointment with our Cedar Point Wellness Clinic, if you are a registered client. Please call (801) 486-4877 to schedule an appointment.

Eye vouchers are issued to clients every 12 months to obtain an eye exam, frames, and plastic lenses. A wellness check at our Cedar Point Wellness Clinic is required to receive an eyeglass voucher. Please call (801) 486-7877 to schedule an eye voucher appointment with one of our medical providers.

Yes, UICSL is an Urban Indian Organization and IHS Title V contract organization.

UICSL at this time does not have funding to provide clients needing access to a Purchased/Referred Care program with Indian Health Services.

Registered clients can schedule an appointment with our Case Manager or Benefits Coordinator to assist with applying for assistance. Please call (801) 486-4877 to schedule an appointment.

For medical bills, please contact the financial assistance office of the hospital or medical office that is billing you to apply for financial assistance. The phone number can usually be found on the bill you receive in the mail.

UICSL does not pay for the cost of dentures, partials, wisdom teeth extraction or orthodontics (braces). We partner with Community Health Centers, Inc. to provide preventative dental care services (cleanings, x-rays, fillings, and simple extractions) to established UICSL clients. Anything out of this scope of care will require the cost of services to be paid by the patient.

These services are available by referral from our Referrals office during office hours.

From time to time, assistance for registered clients is available for some types of major dental work. During these times funds are limited and the process will require an application and income verification documentation in order to provide a prior authorization for major dental work. Please call to speak to a Referral Specialist if a dental assistance program is currently available.

If you arrive 15 minutes or later to your scheduled appointment you may be asked to be reschedule for another day. Please plan to arrive 5-10 minutes before all your UICSL appointments to verify and update paperwork when you check-in.

We will accommodate the needs of clients that come to UICSL as much as possible. If we are unable to assist with your needs in 15 minutes or less you will need to schedule an appointment for another day or return the next morning for services.

Due to the pandemic, events such as powwows and other large gatherings are on hold. 
We do continue to have some socially distanced events for clients if we can do so safely.
