COVID-19 Emergency Assistance

The COVID-19 Emergency Assistance Program is made through a grant received by the Utah Department of Health to be exclusively used to meet reasonable or necessary health expenses which are incurred as a result of the coronavirus pandemic, including but not limited to medical, housing, food, and transportation for medical appointments. Click the link below to request assistance.

DOCUMENTS NEEDED

 

  • Paystubs
  • Documentation of Unemployment Benefits due to COVID
  • Layoff or termination of employment letter due to COVID or contact information for Human Resources
  • Bill from the utility, landlord, mortgage, etc. documenting the amount requested. All supporting documentation must be provided within 30 days of receiving application or it will not be considered for approval.

IMPORTANT INFORMATION

 

COVID-19 EMERGENCY ASSISTANCE FUNDING IS LIMITED AND WILL BE AWARDED TO APPLICANTS WHO CAN PROVIDE DOCUMENTATION SHOWING THAT THE ASSISTANCE REQUESTED IS RELATED TO FINANCIAL HARDSHIP EXPERIENCED DUE TO COVID-19 RESTRICTIONS. ONCE FUNDING IS EXHAUSTED NO APPLICANTS WILL BE ACCEPTED OR APPROVED.

Applications are considered COMPLETE when all documentation has been attached

One application per household address will be accepted

IMPORTANT INFORMATION

COVID-19 Emergency Assistance funding is made through a grant received by the Utah Department of Health. Funding is LIMITED and will be awarded to applicants who can provide documentation showing that the assistance requested is related to a direct-financial hardship experienced due to COVID-19 restrictions. Approved funding is a one-time only award. Amount awarded may not equal amount requested, due to funding guidelines and limited funding. Some requests may be referred to other assistance in the community.
