Children’s Pfizer COVID-19 Vaccine is Here!
Ages 5-11 years old.
UICSL will provide the vaccine Friday, November 12th from 9:30 AM – 4 PM at 120 West 1300 South.
By Appointment Only!

Pfizer Vaccine is available for new and established UICSL members as well as anyone living in the same household.
$25 gift cards will be provided to vaccine recipients.
Parent or legal gaurdian of child(ren) are required to be present during vaccinations.
We have updated our protocol for safety reasons and will need to have parent or legal gaurdian fill out a Minor Consent form.
