Community Needs Assessment Survey

The UICSL is inviting youth individuals (ages 10-17) who self-identify as American Indian and/or Alaska Native and live in the following counties: Davis, Salt Lake, Tooele, Utah, or Weber to take part in the Youth Community Needs Assessment 2020 Study.

Hidden Accordion

Every three to five years, the UICSL asks for community feedback to identify areas of improvement for youth and adult programming. With this data, our UICSL Board of Directors, in partnership with agency staff, can map out a course for health improvement by creating strategies to make positive and sustainable changes in our Urban Indian community of the Salt Lake City front range. If you choose to participate in our 2020 study, all of your responses to the survey questions will be kept confidential and anonymous. The COVID-19 section of the survey will ask questions regarding your health behavior since March 2020 to help us understand your emotional, mental, and physical needs during this time.

The survey results will be used to strengthen, expand, or improve current UICSL programs or potentially develop new ones for the AIAN community.
